[摘要] 目的 检测肺结核患者肺泡灌洗液(bronchoalveolar lavage fluid,BALF)及外周血(peripheral blood,PB)中T淋巴细胞亚群,探讨其在肺结核诊断中的价值.方法 选取47例肺结核患者(观察组)及21例健康体检者(对照组)为研究对象,运用流式细胞仪检测其BALF及外周血中T细胞亚群,并比较其差异性.结果 观察组患者BALF及外周血中CD4 比例及CD4/CD8比值均明显低于对照组(P<0.05);观察组患者BALF中CD8比例显著高于外周血(P<0.05).肺结核患者经治疗2个月后,BALF及外周血中CD4 比例及CD4/CD8比值均有明显升高,BALF中CD8水平与治疗前相比明显降低(P<0.05).绘制ROC曲线比较显示,BALF中T淋巴细胞亚群检测价值高于外周血,差异有统计学意义(P<0.05).结论 BALF及外周血中T淋巴细胞亚群检测有助于肺结核的诊断及观察疗效. |

[Abstract]Objective To evaluate the clinical value of T lymphocyte subsets analysis in bronchoalveolar lavage fluid (BALF) and peripheral blood (PB) in patients with pulmonary tuberculosis. Methods Forty-seven patients with pulmonary tuberculosis (observation group) and 21 normal persons (control group) were enrolled in this study. T lymphocyte subsets in BALF and PB of patients were detected by flow cytometry (FCM), and the differences were compared. Results The CD4 and CD4/CD8 ratios in BALF and PB of observation group were significantly lower than control group (P<0.05), and CD8 in observation group was significantly higher in BALF than in PB (P<0.05). After 2 months' treatments, the CD4 and CD4/CD8 ratios in BALF and PB of observation group were significantly increase,and the CD8 in BALF was reduced (P<0.05). T lymphocyte subsets in BALF showed significantly higher clinical value in diagnosis of pulmonary tuberculosis than that in PB (P<0.05), after ROC curve analysis. Conclusion Analysis of T lymphocyte subsets in BALF and PB is helpful for clinic diagnosis and observation of patients with pulmonary tuberculosis. |
