| ［摘要］目的 探讨非生物型人工肝支持系统联合中药结肠透析治疗肝衰竭对患者血清TNF-α，IL-6及内毒素水平的影响及临床疗效．方法 200例肝衰竭患者随机分为2组，治疗组和对照组各100例，2组均接受非生物型人工肝治疗及内科综合治疗；治疗组则在此基础上根据患者具体情况，联合不同中药组方进行结肠透析，每日一次．2组分别于治疗前、治疗1周、2周、4周，采用流式细胞术检测患者血清TNF-α和IL-6水平，采用动态浊度法检测患者血清内毒素水平．统计分析数据，比较2组间和组内患者血清TNF-α，IL-6及内毒素水平．结果 与对照组比较，治疗4周后治疗组血清TNF-α、IL-6及内毒素水平明显降低，差异具有统计学意义（P＜0.05）；治疗组总有效率85%，对照组总有效率60%，治疗组疗效明显优于对照组，差异具有统计学意义（P＜0.05）；2组患者血清TNF-α水平与血清内毒素水平均呈显著性正相关（分别为r＝0.977，P＝0.023；r＝0.962，P＝0.038）．对照组患者血清IL-6水平与血清内毒素水平呈显著性正相关（r＝0.967，P＝0.033）．结论 非生物型人工肝联合中药结肠透析治疗肝衰竭可明显降低患者血清TNF-α、IL-6及内毒素水平，提高临床疗效. |

| ［Abstract］Objective To study effect of non-biologic artificial liver support systems with Chinese herbs colon dialysis on the serum levels of tumor necrosis factor-alpha，interleukin-6 and endotoxin in the treatment of liver failure. Methods Two hundred patients with hepatic failure，including the treatment group 100 cases and the control group 100 cases. Patients in the two groups received non-biologic artificial liver support systems and internal medicine therapy，patients in the treatment group were also given Chinese herbs colon dialysis with the different Chinese herbal formulas once every day on this basis. The serum TNF-α and IL-6 levels were determined by flow cytometer and serum endotoxin levels were determined by turbidimetric-kinetic method before treatment，1 weeks， 2 weeks，4 weeks after treatment. The differences in serum levels of TNF-α，IL-6 and endotoxin were compared between two groups. Results Compared with control group，the levels of serum TNF-α， IL-6 and endotoxin significantly decreased in patients 4 weeks after treatment in the treatment group（P＜0.05）. The total effective rate of the treatment group was 85%，the total effective rate of the control group was 60%.The therapeutic effect of the treatment group was significantly better than that of the control group （P＜0.05）. A significant positive correlation was found between serum TNF-α levels and serum endotoxin level（the treatment group: r＝0.977，P＝0.023；the control group： r＝0.962，P＝0.038）. A significant positive correlation was found between serum IL-6 levels and serum endotoxin level in the control group（ r＝0.967，P＝0.033）. Conclusion Non-biologic artificial liver support systems with Chinese herbs colon dialysis can significantly decrease the serum levels of TNF-α，IL-6 and endotoxin of patients with hepatic failure，and improve the clinical efficacy. |
